Yes, you must to upload the video manually on Instagram if you want to add a cover photo. If you want a video frame to add a cover to your video, you can schedule the post and Metricool will publish it automatically.

I don’t know how to upload the cover photo to the video. You mention it, but I can’t see how to add the cover photo after uploading the video to the planner.

So far I have been uploading it later on Instagram, which is a bit of a hassle.
